折叠符合性声明
产品负责方(制造商或进口商)将产品在FCC指定的合格检测机构对产品进行检测，做出检测报告，若产品符合FCC标准，则在产品上加贴相应标签，在用户使用手册中声明有关符合FCC标准规定，并保留检测报告以备FCC索要。

办证资料
申请认证产品的生产厂商和申请认证方的全称和详细的联系通信地址。

将提供给用户的认证产品的安装和使用手册的副本。(如该产品还没有用户手册，则可提供相关内容的草稿副本)

产品电气原理图及工作原理说明。(如产品有接地或天线，应加以描述)

有关产品的工作振荡频率表，表中应列出信号的传播路径和相应振荡频率。

其它一些需要说明的产品特点。

备注说明:

1、相关的文件资料需为中英文两种。

2、为缩短认证周期，提供的资料最好为电子文档形式。

3、在认证过程中，针对一些特殊情况，可能需要企业补交其它额外相关资料。

4、申请FCC ID报告做好了需要在FCC ID的网上注册

5、产品说明书必需有警告语
